i AM A RHIT  WHO PRIMARILY DOES INPATIENT CODING.  I HAVE MEDICARE AS MY PRIMARY INSURANCE. I HAVE NEVER SMOKED A CIGARETTE IN MY LIFE.  I AM SEEING A PODIATRIST.  MY DOCTOR SUBMITTED 4004F AS A SECONDARY PROCEDURE CODE.  THE CODE STATES "PATIENT SCREENED FOR TOBACCO USE AND RECEIVED TOBACCO CESSATION INTERVENTION (COUNSELING, PHARMACOTHERAPY, OR BOTH), IF IDENTIFIED AS A SMOKER (PV,CAD).  i DO NOT HAVE CORONARY ARTERY DISEASE AND MY MD HAS NOT STATED I HAVE PERIPHERAL VASCULAR DISEASE.  I WANT TO KNOW IF THIS CODE WAS USED CORRECTLY.

  • The code was not used correctly if there was no discussion with the provider about smoking.  Answering the question on a form does not constitute a dialogue with the provider.
