meaningful use and cause of death

How are others capturing "preliminary cause of death" to meet meaningful
use. Who is documenting this and how is this documented in your EMR? I've
read where some are utilizing abstractors to find this documentation and
entering this into the EMR.

    My coders actually utilize the death certificate when they are coding
    and mark the DX that was the preliminary cause of death in our
    coding/abstracting system. Then daily this information is uploaded into
    our E H R...

    This is also similar to what we are doing by added a second modifier to the diagnosis (like a second poa field) to signify the cause of death from our death progress note. Thanks

    Our hospital does not obtain a copy of the death certificate. Often the funeral home sends it directly to the physician and bypasses HIM department. We are not meeting this objective at this time. What other options would you consider if you didn't have a death certificate?

    In CT the death certificate was copied before it was released to the funeral home or medical examiner. We have the cause of death as stated on the death certificate. The state laws required a copy in the medical record. Do any of you having this problem checked with the your state laws for these requirements?
